JOB SUMMARY
Works with the relevant healthcare providers from the primary to acute care setting, ensuring the continuum of chronic illness care is established. This would include supporting the LIVE Program Director, Clinical Champions and Lead Case Manager., Successful candidates will case manage, co-ordinate and operationalise the initiatives to improve CAD patient care plan. This position would include the participation and co-ordination of LIVE initiatives as a member of the Healthcare Group Coronary Disease Management Team.
MAIN DUTIES AND RESPONSIBILITIES
As a Case Manager, you will
• Review and collate all relevant clinical, medical, psychosocial data as part of initial assessment.
• Identify all patients admitted to the hospital with diagnosis of CAD,
• Ensure that identified patient’s cholesterol tests done, see if patient is hypertensive according to guidelines (i.e inclusion criteria)
Check that all patients put on lipid-lowering Rx / antihypertensive etc
• Ensure all patients admitted to The Health Institute with CHD-related diagnoses achieve optimal control of hypertension and
hypercholesterolaemia in the most cost-effective way and counseled regarding the importance of risk factor control.
Will need to get involved in patient education (complement Cardiac Rehab nurses, if hospital already has them, if not may need to double up)
• See inpatients and outpatients for AMI, angina (stable and unstable), balloon angioplasty, coronary artery bypass surgery, CHD as established by angiography, or non-invasive tests (treadmill stress testing, nuclear or echo-stress imaging) that are being treated medically, will be included in the programme.
• Enlist your patient’s cooperation in developing a treatment plan and according to clinical pathways. You will also mobilize the relevant resources and services your patients require.
• Work within a multi-disciplinary setting, and will be required to communicate and consult with physicians, nurses, and allied health workers. You may be responsible for both inpatient and outpatient services.
• Collate patient’s attendances at SOPD; verify outpatient cholesterol results, blood pressure measurements, titration of cholesterol medications, re-measurement of cholesterol after titration, titration to targets, when patient needs to be on rosuvastation / Ezetimibe etc.
• Communicate regularly with physicians re results and remedial action required
• Document your patient’s progress in case notes and other hospital records, and communicate with internal and external agencies that are involved in the care of your patient.
• Conduct individual and group therapy programs, and work with families.
• Provide training, deliver public information/education presentations, and assist the department in evaluating and improving patient
services.
• Identifying and monitoring high-risk patients and following up on their progress via phone and arrange for evaluation of Lipid
Monitoring Level 6 to 7 weeks post discharged.
• Appropriate establish step-down care of stable patients after 12 -24 months to GPs.
Emphasize that the successful candidate will have a specific work-flow process to follow and that there will be definite end-points we are tracking
• Work with the disease management team on patient care and participate in inter-disciplinary care. Assist in developing and reviewing care plan, admissions and discharges, as well as work on audits and other projects assigned by the Programme Director.
• Ensuring that report deadlines are met and outcomes are achieved as planned.
• Complete audits to assess the compliance of CAD clinical practice guidelines, and participate in clinical practice improvement
projects.
• Participate and contribute to the development of patient educational materials for patients with CAD.
