The Challenges
You will oversee the transition of patients from hospitals’ specialist clinics to primary and community care. You will be responsible for the coordination, monitoring and tracking of patients to ensure continuity of care upon discharge, and will collaborate with a multi-disciplinary team of doctors, nurses, medical social workers and para-medicals from the clinics to ensure the smooth transition and right sitting of patients to primary care.
In addition, you will also be expected to take on ad-hoc projects to help improve the process of healthcare delivery across the care continuum. As part of your job, you will also provide financial counseling to patients and perform simple data and statistics management.
Requirements
• Diploma in Nursing
• Prior experience in healthcare will be an added advantage
• Possess excellent communication and counseling skills
• Proficiency in Microsoft Office applications
• Singapore Citizen / SPR
Working location: Hospital located at Central /West
